Artificial intelligence to now help diagnose breast cancer

Washington D.C., Aug 10: Researchers discovered an artificial intelligence system that could help pathologists read biopsies more accurately and to better detect and diagnose breast cancer. 

The new system, described in a study published in the journal 'JAMA Network Open,' helped interpret medical images used to diagnose breast cancer that can be difficult for the human eye to classify, and it does so nearly as accurate or better as experienced pathologists.

"It is critical to get a correct diagnosis from the beginning so that we can guide patients to the most effective treatments," said Dr. Joann Elmore, the study's senior author and a professor of medicine at the David Geffen School of Medicine at UCLA.

A 2015 study led by Elmore found that pathologists often disagree on the interpretation of breast biopsies, which are performed on millions of women each year.

That earlier research revealed that diagnostic errors occurred in about one out of every six women who had ductal carcinoma in situ (a noninvasive type of breast cancer), and that incorrect diagnoses were given in about half of the biopsy cases of breast atypia (abnormal cells that are associated with a higher risk for breast cancer).

"Medical images of breast biopsies contain a great deal of complex data and interpreting them can be very subjective. Distinguishing breast atypia from ductal carcinoma in situ is important clinically but very challenging for pathologists. Sometimes, doctors do not even agree with their previous diagnosis when they are shown the same case a year later," said Elmore, who is also a researcher at the UCLA Jonsson Comprehensive Cancer Center.

The scientists reasoned that artificial intelligence could provide more accurate readings consistently because by drawing from a large data set, the system can recognise patterns in the samples that are associated with cancer but are difficult for humans to see.

The team fed 240 breast biopsy images into a computer, training it to recognise patterns associated with several types of breast lesions, ranging from benign (noncancerous) and atypia to ductal carcinoma in situ, or DCIS, and invasive breast cancer. Separately, the correct diagnoses for each image were determined by a consensus among three expert pathologists.

To test the system, the researchers compared its readings to independent diagnoses made by 87 practising U.S. pathologists. While the artificial intelligence program came close to performing as well as human doctors in differentiating cancer from non-cancer cases, the AI program outperformed doctors when differentiating DCIS from atypia, considered the greatest challenge in breast cancer diagnosis.

The system correctly determined whether scans showed DCIS or atypia more often than the doctors; it had sensitivity between 0.88 and 0.89, while the pathologists' average sensitivity was 0.70. (A higher sensitivity score indicates a greater likelihood that diagnosis and classification are correct.)

"These results are very encouraging. There is low accuracy among practising pathologists in the U.S. when it comes to the diagnosis of atypia and ductal carcinoma in situ, and the computer-based automated approach shows great promise," Elmore said.

There is no evidence that the Bacille Calmette-Guerin (BCG) vaccine, which is primarily used against tuberculosis, protects people against infection with the novel coronavirus, the World Health Organization (WHO) said.

The WHO therefore didn't recommend BCG vaccination for the prevention of COVID-19 in the absence of evidence, according to its daily situation report on Monday, Xinhua news agency reported.

"There is experimental evidence from both animal and human studies that the BCG vaccine has non-specific effects on the immune system. These effects have not been well characterized and their clinical relevance remains unknown," WHO stated.

Two clinical trials addressing the question are underway, and WHO will evaluate the evidence when it is available, it noted.

BCG vaccination prevents severe forms of tuberculosis in children and diversion of local supplies may result in an increase of disease and deaths from the tuberculosis, it warned.

Mar 6: The spread of the new coronavirus is shining the spotlight on a little-discussed gender split: men wash their hands after using the bathroom less than women, years of research and on-the-ground observations show.

Health officials around the world advise that deliberate, regular handwashing is one of the best weapons against the virus which causes a flu-like respiratory illness that can kill and has spread to around 80 countries.

The Centers for Disease Control and Prevention's online fact sheet "Handwashing: A corporate activity," cites a 2009 study that finds "only 31% of men and 65% of women washed their hands" after using a public restroom.

Social media comments about men's handwashing lapses forced an august British institution to caution visitors about bathroom behaviour this week.

After author Sathnam Sanghera complained on Twitter about "grown," "educated" men in the British Library toilets not washing their hands, the library responded, putting up additional signs reminding patrons to wash their hands in men's and women's bathrooms.

Thanks to "visitor feedback," a spokesman told Reuters, "we have increased further the number of posters in public toilets so that visitors are reminded of the importance of good hygiene at exactly the point where they can wash their hands."

Men and women approach handwashing after using the restroom differently, according to multiple surveys and field studies.

"Women wash their hands significantly more often, use soap more often, and wash their hands somewhat longer than men," according to a 2013 Michigan State University field study conducted by research assistants who observed nearly 4,000 people in restrooms around East Lansing, Michigan.

The study found 14.6% of men did not wash their hands at all after using the bathroom and 35.1% wet their hands but did not use soap, compared to 7.1% and 15.1% of women, respectively.

"If you stand in the men's bathroom at work, and watch men leave, they mostly don't wash their hands if they used the urinal," said one New York City public relations executive, who did not want to be identified for fear of alienating his colleagues.

Since the virus's spread, he's seen an uptick in men's handwashing at work, he noted. "I, for the record, do wash my hands all the time," he added.

Female medical staff in critical care units "washed their hands significantly more often than did their male counterparts after patient contact," a 2001 study published in the American Journal of Infection Control found.

Middle-aged women with some college education had the highest level of knowledge about hand hygiene, a survey published in 2019 by BMC Public Health, an open access public health journal, found.

Early information about coronavirus infection in China shows that men may be more susceptible to the disease. Just over 58% of the more than 1,000 COVID-19 patients reported in China through Jan. 29, 2020, were male, research published in the New England Journal of Medicine shows.

Researchers have not linked the difference to hand hygiene.

Tedros Adhanom Ghebreyesus, the World Health Organisation's (WHO) Director-General, said that a clinical trial of hydroxychloroquine (HCQ) on COVID-19 patients has come to "a temporary pause", while the safety data of the the anti-malaria drug was being reviewed.

According to the WHO chief, The Lancet medical journal on May 22 had published an observational study on HCQ and chloroquine and its effects on COVID-19 patients that have been hospitalized, reports Xinhua news agency.

The authors of the study reported that among patients receiving the drug, when used alone or with a macrolide, they estimated a higher mortality rate.

"The Executive Group of the Solidarity Trial, representing 10 of the participating countries, met on Saturday (May 23) and has agreed to review a comprehensive analysis and critical appraisal of all evidence available globally," Tedros said in a virtual press conference on Monday.

The review will consider data collected so far in the Solidarity Trial and in particular robust randomized available data, to adequately evaluate the potential benefits and harms from this drug, he said.

"The Executive Group has implemented a temporary pause of the HCQ arm within the Solidarity Trial while the safety data is reviewed by the Data Safety Monitoring Board. The other arms of the trial are continuing," Tedros added.

WHO initiated the Solidarity Trial, a plan to evaluate the safety and efficacy of four drugs and drug combinations against COVID-19 more than two months ago, which include HCQ.

According to the WHO, over 400 hospitals in 35 countries are actively recruiting patients and nearly 3,500 patients have been enrolled from 17 countries under the Solidarity Trial.

Tedros added that the safety concern over the drug related only to the use of HCQ and chloroquine in COVID-19, and "these drugs are accepted as generally safe for use in patients with autoimmune diseases or malaria".

"WHO will provide further updates as we know more," he added.
