India's first double hand transplant successful, says doctors

Thiruvananthapuram, Feb 1: With the first successful double hand transplant, Indian doctorshave registered a rare feat of carrying out the world's first hand transplant of a coloured skin and the first hand transplant in any developing country.

So far, only 110 successful hand transplants have been done, since the first one in France 13 years ago, in USA, European countries, China and Australia.

The honour for the rare feat goes to a team of surgeons at the Kochi-based Amrita Institute of Medical Sciences. The transplant was carried out on January 12 and 13 to a train accident victim - 30-year-old Manu, who received both the hands of a 24-year-old road accident victim.

"The operation lasted for 16 hours with more than 20 surgeons participating in the procedure. Each hand required connecting two bones, two arteries, four veins and about 14 tendons," saidDr. Subramania Iyer, Professor and Head of the Plastic Surgery Department.

"The immune suppressant drugs were started before the start of the surgery and continued after it. He was shifted to a transplant ICU. Careful monitoring of the blood supply to the hands was done and all precautions to prevent infections was instituted in the nursing. Another operation to replace an area of bad skin was done for his left hand on the second day," he added.

Dr. Iyer further said, "After14 days post-surgery, both the hands have been accepted by the recipient's body and he started regaining movements. He has been shifted out of the ICU and post-surgical rehabilitation process has started under supervision."

"Manu will be able to move his hands since his own muscles of the forearm are working to move the fingers. The sensations will return only slowly and is expected in three to four months. His immunosuppressant drugs (to prevent rejection of the transplanted hands) will have to be continued indefinitely, but at a reduced rate from three months," he added.

Dr. Iyer said, the current transplant is important, since follow up of the transplanted skin is done by the appearance of the skin and skin biopsies every month to see signs of rejection.

He said the details of the transplant have been added to the International Registry of Hand Transplants, which maintains stringent standards in the follow-up of these cases.

The entire expense for the transplant was borne by Mata Amritanandamayiu Math.

"This transplant has put the Indian scientific community among the league of a few developed nations in the field of composite tissue allotransplant," said Dr. Iyer.

Washington, Aug 2: Children under the age of five have between 10 to 100 times greater levels of genetic material of the coronavirus in their noses compared to older children and adults, a study in JAMA Pediatrics said Thursday.

Its authors wrote this meant that young children might be important drivers of Covid-19 transmission within communities -- a suggestion at odds with the current prevailing narrative.

The paper comes as the administration of US President Donald Trump is pushing hard for schools and daycare to reopen in order to kickstart the economy.

Between March 23 and April 27, researchers carried out nasal swab tests on 145 Chicago patients with mild to moderate illness within one week of symptom onset.

The patients were divided into three groups: 46 children younger than five-years-old, 51 children aged five to 17 years, and 48 adults aged 18 to 65 years.

The team, led by Dr Taylor Heald-Sargent of the Ann & Robert H. Lurie Children's Hospital, observed, "a 10-fold to 100-fold greater amount of SARS-CoV-2 in the upper respiratory tract of young children."

The authors added that a recent lab study had demonstrated that the more viral genetic material was present, the more infectious virus could be grown.

It has also previously been shown that children with high viral loads of the respiratory syncytial virus (RSV) are more likely to spread the disease.

"Thus, young children can potentially be important drivers of SARS-CoV-2 spread in the general population," the authors wrote.

"Behavioral habits of young children and close quarters in school and daycare settings raise concern for SARS-CoV-2 amplification in this population as public health restrictions are eased," they concluded.

The new findings are at odds with the current view among health authorities that young children -- who, it has been well established, are far less likely to fall seriously ill from the virus -- don't spread it much to others either.

However, there has been fairly little research on the topic so far.

One recent study in South Korea found children aged 10 to 19 transmitted Covid-19 within households as much as adults, but children under nine transmitted the virus at lower rates.

The record levels of new daily COVID-19 cases are due to the fact that the pandemic is peaking in a number of big countries at the same time and reflect a change in the virus' global activity, the World Health Organisation said.

At a media briefing on Monday, WHO's emergencies chief Dr Michael Ryan said that the numbers are increasing because the epidemic is developing in a number of populous countries at the same time.

Some countries have attributed their increased caseload to more testing, including India and the US But Ryan dismissed that explanation.

We do not believe this is a testing phenomenon, he said, noting that numerous countries have also noted marked increases in hospital admissions and deaths neither of which cannot be explained by increased testing.

There definitely is a shift in that the virus is now very well established, Ryan said. The epidemic is now peaking or moving towards a peak in a number of large countries.

He added the situation was definitely accelerating in a number of countries, including the US and others in South Asia, the Middle East and Africa.

Washington D.C., Apr 4: While consuming a high-diet salt can result in high blood pressure, a recent study has revealed a link between salt-rich diet and weaker immune system.

The study was conducted under the leadership of the University Hospital Bonn, and the results were published in the journal Science Translational Medicine.

The research was conducted on mice that were fed a high-salt diet. Later, they were found to suffer from much more severe bacterial infections.

Human volunteers who consumed an additional six grams of salt per day also showed pronounced immune deficiencies.

The World Health Organization (WHO) has recommended a maximum amount of five grams of salt a day.

It corresponds approximately to one level teaspoon. In reality, however, many Germans exceed this limit considerably. 

Figures from the Robert Koch Institute suggest that on average men consume ten, and women more than eight grams a day.

This means that we reach for the salt shaker much more than is good for us. After all, sodium chloride, which is its chemical name, raises blood pressure and thereby increases the risk of heart attack or stroke.

"We have now been able to prove for the first time that excessive salt intake also significantly weakens an important arm of the immune system," said Prof. Dr. Christian Kurts from the Institute of Experimental Immunology at the University of Bonn.

This finding is unexpected, as some studies point in the opposite direction. For example, infections with certain skin parasites in laboratory animals heal significantly faster if these consume a high-salt diet.

The study also sheds light on the fact that the skin serves as a salt reservoir.

"Our results show that this generalization is not accurate," emphasized Katarzyna Jobin, lead author of the study.

The body keeps the salt concentration in the blood and in the various organs largely constant. Otherwise important biological processes would be impaired. The only major exception is the skin which functions as a salt reservoir of the body. This is why the additional intake of sodium chloride works so well for some skin diseases.

However, other parts of the body are not exposed to the additional salt consumed with food. Instead, it is filtered out by the kidneys and excreted in the urine.

"We examined volunteers who consumed six grams of salt in addition to their daily intake," said Prof. Kurts. This is roughly the amount contained in two fast-food meals, i.e. two burgers and two portions of French fries.

After one week, from the results, it showed that the immune cells coped much worse with bacteria after the test subjects had started to eat a high-salt diet.

In human volunteers, excessive salt intake also resulted in increased glucocorticoid levels.
