Diet sodas might not raise diabetes risk

Nov 16: Drinking colas and other sugary drinks is tied to an increased risk of so-called pre-diabetes, a precursor to full-blown disease, but diet soda is not, a recent study suggests.

dietPrevious studies on the link between diet sodas and diabetes have been mixed; some research pointing to a potential connection has suggested this relationship may be explained at least in part by soda drinkers being overweight or obese.

In the current study, however, adults who routinely consumed at least one can of soda or other sugar-sweetened beverages a day were 46 percent more likely to develop elevated blood sugar levels than people who rarely or never drink cola.

“Emphasis should be placed on substituting sugar-sweetened beverages with water, unsweetened teas, or coffee,” said senior study author Nicola McKeown, a nutrition researcher at Tufts University in Boston.

“For daily consumers of sugary drinks, kicking the habit may be a difficult challenge, and incorporating an occasional diet soda, while increasing fluids from other sources, may be the best strategy to ultimately remove sugar-sweetened beverages from the diet,” McKeown added by email.

Globally, about one in nine adults have diabetes, and the disease will be the seventh leading cause of death by 2030, according to the World Health Organization. Most of these people have Type 2, or adult-onset, diabetes, which happens when the body can't properly use or make enough of the hormone insulin to convert blood sugar into energy.

People with blood sugar levels that are slightly elevated, but not high enough for a diabetes diagnosis, are sometimes described as having “pre-diabetes” because many will go on to develop diabetes. In the current study, researchers examined data collected on 1,685 middle-aged adults over about 14 years.

At the start of the study, none of the participants had diabetes or pre-diabetes. They were 52 years old on average and typically overweight.

Participants completed questionnaires detailing what they ate and drank during the study period. Sugar-sweetened beverages were defined as colas and other carbonated beverages, as well as drinks such as lemonade and fruit punch. This didn’t include fruit juice.

People who drank the most sodas – typically around six 12-ounce cans a week – had a much greater risk of developing elevated blood sugar levels than other participants after adjusting for factors such as age, gender and weight, researchers report in the Journal of Nutrition.

Higher consumption of soda and other sugar-sweetened beverages was also associated with insulin resistance, a reduced ability to respond to the hormone insulin that is another risk factor for developing diabetes.

Even after accounting for changes in weight and other aspects of diet, the relationships between sugar-sweetened beverages and these metabolic risk factors for diabetes persisted.

Diet soda intake—defined as low-calorie cola or other carbonated low-calorie beverages— was not associated with elevated blood sugar or insulin resistance.

The study doesn’t prove soda or sugary drinks cause diabetes.

Another limitation of the study is that participants may not be representative of a typical U.S. adult, the authors note. People in the study were mostly white, middle aged and more likely to be women. They also tended not to be as overweight or thick around the middle as many U.S. adults, the authors point out.

Because pre-diabetic elevated blood sugar can often be reversed before it advances to full-blown disease, the findings suggest it makes sense for people to avoid regular sodas to minimize their risk of developing diabetes, the researchers conclude.

“Sugar sweetened beverages have been shown to increase weight gain and risk of diabetes – including prediabetes,” Laura Rosella, a public health researcher at the University of Toronto who wasn’t involved in the study, said by email.

The current study findings add to a large body of evidence suggesting that the sugar and calories in soda can contribute to the risk of obesity and diabetes, noted Dr. Robert Cohen, a researcher at the University of Cincinnati College of Medicine who wasn’t involved in the study.

“I wouldn’t necessarily seek out diet drinks but the choice of non-calorie containing diet drinks is not associated with further insulin resistance or pre-diabetes in the way that calorie containing drinks are,” Cohen said by email.

Clinician-scientists have found that Irish patients admitted to hospital with severe coronavirus (COVID-19) infection are experiencing abnormal blood clotting that contributes to death in some patients.

The research team from the Royal College of Surgeons in Ireland found that abnormal blood clotting occurs in Irish patients with severe COVID-19 infection, causing micro-clots within the lungs.

According to the study, they also found that Irish patients with higher levels of blood clotting activity had a significantly worse prognosis and were more likely to require ICU admission.

"Our novel findings demonstrate that COVID-19 is associated with a unique type of blood clotting disorder that is primarily focussed within the lungs and which undoubtedly contributes to the high levels of mortality being seen in patients with COVID-19," said Professor James O'Donnell from St James's Hospital in Ireland.

In addition to pneumonia affecting the small air sacs within the lungs, the research team has also hundreds of small blood clots throughout the lungs.

This scenario is not seen with other types of lung infection and explains why blood oxygen levels fall dramatically in severe COVID-19 infection, the study, published in the British Journal of Haematology said.

"Understanding how these micro-clots are being formed within the lung is critical so that we can develop more effective treatments for our patients, particularly those in high-risk groups," O'Donnell said.

"Further studies will be required to investigate whether different blood-thinning treatments may have a role in selected high-risk patients in order to reduce the risk of clot formation," Professor O'Donnell added.

According to the study, emerging evidence also shows that the abnormal blood-clotting problem in COVID-19 results in a significantly increased risk of heart attacks and strokes.

As of Friday morning, the cases increased to 20,612 cases in Ireland, with 1,232 deaths so far, according to the Johns Hopkins University.

Scientists have designed a “catch and kill” air filter which they say can trap the novel coronavirus and neutralise it instantly, an invention that may reduce the spread of COVID-19 in closed spaces such as schools, hospitals and health care facilities, as well as public transit environments like airplanes.

According to the study, published in the journal Materials Today Physics, the device killed 99.8 per cent of the novel coronavirus, SARS-CoV-2, in a single pass through its filter. It said the device, made from commercially available nickel foam heated to 200 degrees Celsius, also killed 99.9 per cent of the spores of the deadly bacterium Bacillus anthracis which causes the anthrax disease.

“This filter could be useful in airports and in airplanes, in office buildings, schools, and cruise ships to stop the spread of COVID-19,” said Zhifeng Ren, a co-author of the study from the University of Houston (UH) in the US.

“Its ability to help control the spread of the virus could be very useful for society,” Ren added.

The researchers said they are also developing a desk-top model for the device which is capable of purifying the air in an office worker’s immediate surroundings. According to the scientists, since the virus can remain in the air for about three hours, a filter that could remove it quickly was a viable plan, and with businesses reopening across the world, they believe controlling the spread in air conditioned spaces was urgent.

The study noted that the novel coronavirus cannot survive temperatures above 70 degrees Celsius, so by making the filter temperature far hotter — about 200 degree Celsius, the researchers said they were able to kill the virus almost instantly.

Ren said the nickel foam met several key requirements. “It is porous, allowing the flow of air, and electrically conductive, which allowed it to be heated. It is also flexible,” the researchers noted in a statement.But they added that nickel foam also had low resistivity, making it difficult to raise the temperature high enough to quickly kill the virus.

The researchers said they solved this problem by folding the foam, connecting multiple compartments with electrical wires to increase the resistance high enough to raise the temperature as high as 250 degrees Celsius. By making the filter electrically heated, rather than heating it from an external source, they said the the amount of heat that escaped from the filter is minimised, allowing air conditioning to function with very low strain.

When the scientists built and tested a prototype for the relationship between voltage/current and temperature, they said it satisfies the requirements for conventional heating, ventilation, and air conditioning (HVAC) systems, and could kill the coronavirus.

“This novel biodefense indoor air protection technology offers the first-in-line prevention against environmentally mediated transmission of airborne SARS-CoV-2, and will be on the forefront of technologies available to combat the current pandemic and any future airborne biothreats in indoor environments,” said Faisal Cheema, another co-author of the study from UH.

The researchers have called for a phased roll-out of the device, “beginning with high-priority venues, where essential workers are at elevated risk of exposure.” They believe the novel device will both improve safety for frontline workers in essential industries and allow nonessential workers to return to public work spaces.

While coughing, fever and difficulty in breathing are common symptoms of COVID-19, a new case study has found that pink eye is also a reason to be tested for the disease.

The study, published in the Canadian Journal of Ophthalmology, determined that conjunctivitis and keratoconjunctivitis can also be primary symptoms of COVID-19.

The researchers noted that in March, a 29-year-old woman arrived at the Royal Alexandra Hospital's Eye Institute of Alberta with a severe case of conjunctivitis and minimal respiratory symptoms.

After the patient had undergone several days of treatment with little improvement -- and after it had been determined that the woman had recently returned home from Asia -- a resident ordered a COVID-19 test.

The test came back positive, according to the researchers.

"What is interesting in this case, and perhaps very different to how it had been recognised at that specific time, was that the main presentation of the illness was not a respiratory symptom. It was the eye," said Carlos Solarte, an assistant professor at the University of Alberta in Canada.

"There was no fever and no cough, so we weren't led to suspect COVID-19 at the beginning. We didn't know it could present primarily with the eye and not with the lungs," Solarte said.

Academic studies at the outset of the pandemic identified conjunctivitis as a secondary symptoms in about 10 to 15 per cent of COVID-19 cases, he said.

Since then, scientists have gained greater knowledge of how the virus can transmit through and affect the body's mucous membrane system, of which the conjunctiva -- the clear, thin membrane that covers the front surface of the eye -- is an extension.

While the finding provides important new health information for the public, it also makes eye exams more complicated for ophthalmologists and staff, the researchers noted.

"The patient in this case eventually recovered well without any issues. But several of the residents and staff who were in close contact with the patient had to be under quarantine," said Solarte.

"Fortunately, none who were involved in her care also tested positive," he said.

Patients coming into an eye clinic with conjunctivitis and keratoconjunctivitis are now treated as potential cases of COVID-19 and extra precautions are taken by staff, according to the researchers.
