Heart risks linked to ‘food deserts’ caused by costs, not lack of access

Living in an area with little access to fresh and nutritious foods has been linked to high heart disease risk, but a new study suggests that it’s the inability to afford a healthy diet, rather than access, that’s to blame.

Researchers studied Atlanta residents and found that people living in “food deserts,” where there are few places to buy fresh produce and other healthy foods, had more heart risk factors like hardened arteries and inflammation than people with easy access to healthy foods.

But within food-desert neighborhoods, people with high personal income had fewer heart risk factors than those with low incomes, suggesting it’s money, not access, that prevents some people from having a healthy diet that would lower their heart risk, the study team concludes in Circulation: Cardiovascular Quality and Outcomes.

“Food deserts are defined as areas that have below average income together with poor access to healthy foods, ie. lack of grocery stores (within 1 mile in urban and 10 miles in rural communities),” lead author Dr. Arshed Quyyumi told Reuters Health in an email.

“We found that area income, and even more importantly, personal income was associated with higher cardiovascular risk, and that access to food was not that important a risk,” said Quyyumi, a cardiologist at Emory University School of Medicine in Atlanta.

Researchers have known that neighborhood factors are important social determinants of disease outcomes, he added.

For the study, Quyyumi and his colleagues examined data on more than 1,400 adults, averaging about 50 years old, and living in the Atlanta metropolitan area. Just under 40 percent were men and about 37 percent were African American.

The researchers collected personal and economic information and performed tests to detect signs of inflammation, elevated blood sugar and blood pressure, as well as arterial stiffness.

About 13 percent of participants lived in areas considered food deserts. These people also had higher rates of smoking, were more likely to have high blood pressure and hardened arteries and to be overweight or obese, compared to those not living in food deserts.

When the study team took average neighborhood income and individual incomes into consideration, they found that people living in food deserts in low-income areas had about the same risk of heart disease as their peers living in low-income areas with good food access.

Meanwhile, high-income individuals in low-income neighborhoods had fewer cardiovascular risk factors compared to their lower-income neighbors, and that was true even when they lived in food deserts.

“People not having access to healthy food choices is a possible cause for poor health. However, our study shows the greater impact of lower socio-economic status as a stronger risk factor,” Quyyumi said.

The study team was partly surprised to find so little impact from food access, he said, but speculated

New Delhi, Jun 24: Expanding the testing criterion for coronavirus, the Indian Council of Medical Research has said it should be made widely available to all symptomatic individuals across the country.

"Since test, track and treat' is the only way to prevent spread of infection and save lives, it is imperative that testing should be made widely available to all symptomatic individuals in every part of the country and contact tracing mechanisms for containment of infection are further strengthened," it said in an advisory on 'Newer Additional Strategies for COVID-19 Testing' on Tuesday.

In its revised testing strategy for COVID-19 issued on May 18, the Indian Council of Medical Research (ICMR) had advised testing for all symptomatic Influenza-like illness (ILI) among returnees and migrants within seven days of illness.

All hospitalised patients who develop ILI symptoms, symptomatic individuals living within hotspots or containment zones and healthcare and frontline workers involved in containment and mitigation of coronavirus were also advised testing.

The apex health research body has also advised authorities to enable all government and private hospitals, offices and public sector units to perform antibody-based COVID-19 testing for surveillance to help allay fears and anxiety of healthcare workers and office employees.

The earlier advisories on rapid antibody testing advisories had focused on areas reporting clusters (containment zones), large migration gatherings/evacuees centers and testing of symptomatic ILI individuals at facility level.

Besides, the ICMR on Tuesday also recommended deployment of rapid antigen detection tests for COVID-19 in combination with RT-PCR tests in all containment zones, all central and state government medical colleges and government hospitals, all private hospitals approved by the National Accreditation Board for Hospitals and Healthcare (NABH), all NABL-accredited and ICMR approved private labs, for COVID-19 testing.

All hospitals, laboratories and state governments intending to perform the point-of-care antigen tests need to register with ICMR to obtain the login credentials for data entry.

"ICMR advises all state governments, public and private institutions concerned to take required steps to scale up testing for COVID-19 by deploying combination of various tests as advised," the advisory added.

The World Health Organization (WHO) is reviewing a report that suggested its advice on the novel coronavirus needs updating after some scientists told the New York Times there was evidence the virus could be spread by tiny particles in the air.

The WHO says the Covid-19 disease spreads primarily through small droplets, which are expelled from the nose and mouth when an infected person breaths them out in coughs, sneezes, speech or laughter and quickly sink to the ground.

In an open letter to the Geneva-based agency, 239 scientists in 32 countries outlined the evidence they say shows that smaller exhaled particles can infect people who inhale them, the newspaper said on Saturday.

Because those smaller particles can linger in the air longer, the scientists - who plan to publish their findings in a scientific journal this week - are urging WHO to update its guidance, the Times said.

"We are aware of the article and are reviewing its contents with our technical experts," WHO spokesman Tarik Jasarevic said in an email reply on Monday to a Reuters request for comment.

The extent to which the coronavirus can be spread by the so-called airborne or aerosol route - as opposed to by larger droplets in coughs and sneezes - remains disputed.

Any change in the WHO's assessment of the risk of transmission could affect its current advice on keeping one-metre physical distancing. Governments, which also rely on the agency for guidance policy, may also have to adjust public health measures aimed at curbing the spread of the virus.

"Especially in the last couple of months, we have been stating several times that we consider airborne transmission as possible but certainly not supported by solid or even clear evidence," Benedetta Allegranzi, the WHO's technical lead for infection prevention and control, was quoted as saying in the New York Times.

WHO guidance to health workers, dated June 29, says that SARS-CoV-2, the virus that causes Covid-19, is primarily transmitted between people through respiratory droplets and on surfaces.

But airborne transmission via smaller particles is possible in some circumstances, such as when performing intubation and aerosol-generating procedures, it says.

Medical workers performing such procedures should wear heavy-duty N95 respiratory masks and other protective equipment in an adequately ventilated room, the WHO says.

Officials at South Korea's Centers for Disease Control said on Monday they were continuing to discuss various issues about Covid-19, including the possible airborne transmission. They said more investigations and evidence were needed.

New York, Jul 30: Can the coronavirus spread through the air? Yes, it's possible.

The World Health Organisation recently acknowledged the possibility that Covid-19 might be spread in the air under certain conditions.

Recent Covid-19 outbreaks in crowded indoor settings — restaurants, nightclubs and choir practices — suggest the virus can hang around in the air long enough to potentially infect others if social distancing measures are not strictly enforced.

Experts say the lack of ventilation in these situations is thought to have contributed to spread, and might have allowed the virus to linger in the air longer than normal.

In a report published in May, researchers found that talking produced respiratory droplets that could remain in the air in a closed environment for about eight to 14 minutes.

The WHO says those most at risk from airborne spread are doctors and nurses who perform specialized procedures such as inserting a breathing tube or putting patients on a ventilator.

Medical authorities recommend the use of protective masks and other equipment when doing such procedures.

Scientists maintain it's far less risky to be outside than indoors because virus droplets disperse in the fresh air, reducing the chances of Covid-19 transmission.
