High BP in late life may protect against dementia: study

Los Angeles, Jan 17: Onset of high blood pressure later in life, especially at age 80 or older, is associated with lower dementia risk after 90, a new study has found. High blood pressure and other heart health risk factors are generally thought to increase dementia risk.

bpResearchers challenge this idea and add to scientist's understanding of hypertension and dementia risk over a person's life course.

Researchers at the University of California, Irvine in the US followed 559 people for an average of 2.8 years to investigate the relationship between dementia, age of hypertension onset and blood pressure measurements. All participants are from an ongoing, long-term study of people aged 90 and older known as 'The 90+ Study'. At enrolment, participants did not have dementia, were 93 years old on average and 69 per cent female.

They received dementia assessments every six months during the study period. During the follow-up period, 224 (40 per cent) of the participants were diagnosed with dementia. The researchers found that study participants who reported hypertension onset at age 80 to 89 were 42 per cent less likely to develop dementia after age 90 compared to those who reported no history of high blood pressure.

Participants whose hypertension began at age 90 or older were at even lower risk — 63 per cent less likely to develop dementia. These associations were statistically significant and independent of whether participants were taking medications to treat hypertension. "In this first-of-its-kind study, we find that hypertension is not a risk factor for dementia in people aged 90 or over but is actually associated with reduced dementia risk," said Maria Corrada, professor at University of California.

"This relationship had not yet been examined in groups of older people in their 80s or 90s, known as the 'oldest old'," said Corrada. The researchers also measured study participants' blood pressure at enrolment. Those in the hypertensive range at baseline were at lower risk for dementia compared to those with blood pressure in the normal range.

While these results were not statistically significant, the researchers observed that dementia risk declined as hypertension severity increased - a trend consistent with the idea that, in this age group, hypertension may protect the brain from insults that lead to dementia. The researchers suggest several potential reasons for the association between hypertension and dementia risk observed in the study.

These include that blood pressure may need to reach a certain level to maintain adequate blood flow in the brain for normal cognition and that this level may change with age. Another explanation that the researchers note is less likely, but possible, is that blood pressure drops before the onset of dementia as a consequence of brain cell deterioration and thus older people who are not developing dementia will have higher blood pressure. The study was published in the journal Alzheimer's and Dementia.

Researchers have found the rates of lung cancer are higher in young women than men.

The study, published in the journal Pediatrics, examined lung cancer rates in young adults in 40 countries across five continents and uncovered a trend of higher lung cancer rates in women compared with men in recent years.

The emerging trend was widespread, affecting countries across varied geographic locations and income levels.

The changes appeared to be driven by a rising rate of adenocarcinoma lung cancer among women, said the study researchers from University of Calgary in Canada.

Lung cancer rates have been higher among men than women because men started smoking in large numbers earlier and smoked at higher rates; however, recent studies have reported converging lung cancer incidence rates between sexes.

Among men, age specific lung cancer incidence rates generally decreased in all countries, while in women the rates varied across countries with the trends in most countries stable or declining, albeit at a slower pace compared to those in men.

For the findings, lung and bronchial cancer cases between 30-64 age group from 1993-2012 were extracted from cancer incidence in five continents.

The study found the higher emerging rates of lung cancer in young women compared to young men.

According to the researchers, future studies are needed to identify reasons for the elevated incidence of lung cancer among young women.

Lower neighbourhood socioeconomic status and greater household crowding increase the risk of becoming infected with SARS-CoV-2, the virus that causes COVID-19, warn researchers.

"Our study shows that neighbourhood socioeconomic status and household crowding are strongly associated with risk of infection," said study lead author Alexander Melamed from Columbia University in the US.

"This may explain why Black and Hispanic people living in these neighbourhoods are disproportionately at risk for contracting the virus," Melamed added.

For the findings, published in the journal JAMA, the researchers examined the relationships between COVID-19 infection and neighbourhood characteristics in 396 women who gave birth during the peak of the Covid-19 outbreak in New York City. Since March 22, all women admitted to the hospitals for delivery have been tested for the virus, which gave the researchers the opportunity to detect all infections -- including infections with no symptoms -- in a defined population

The strongest predictor of COVID-19 infection among these women was residence in a neighbourhood where households with many people are common.The findings showed that women who lived in a neighbourhood with high household membership were three times more likely to be infected with the virus. Neighbourhood poverty also appeared to be a factor, the researchers said.Women were twice as likely to get COVID-19 if they lived in neighbourhoods with a high poverty rate, although that relationship was not statistically significant due to the small sample size.

The study revealed that there was no association between infection and population density.

"New York City has the highest population density of any city in the US, but our study found that the risks are related more to density in people's domestic environments rather than density in the city or within neighbourhoods," says co-author Cynthia Gyamfi-Bannerman."

The knowledge that SARS-CoV-2 infection rates are higher in disadvantaged neighbourhoods and among people who live in crowded households could help public health officials target preventive measures," the authors wrote.

Recently, another study published in the Journal of the American Planning Association, showed that dense areas were associated with lower COVID-19 death rates.

New Delhi, Mar 27: The Centre has restricted sale and distribution of "hydroxychloroquine" declaring it as an essential drug to treat the COVID-19 patients and meet the requirements of emergency arising due to the pandemic.

The Ministry of Health and Family Welfare on Thursday made the announcement making it clear that the order "shall come into force on the date of its publication in the official Gazette".

In the order, the government declared that the Central government is "satisfied that the drug hydroxychloroquine is essential to meet the requirements of emergency arising due to pandemic COVID-19 and in the public interest, it is necessary and expedient to regulate and restrict the sale and distribution of the drug 'hydroxychloroquine' and preparation based thereon for preventing their misuse".

"Now, therefore, in exercise of the powers conferred by Section 26B of the Drugs and Cosmetics Act, 1940 (23 of 1940), the Central government hereby directs that sale by retail of any preparation containing the drug Hydroxychloroquine shall be in accordance with the conditions for sale of drugs specified in Schedule H1 to the Drugs and Cosmetics Rules, 1945."

The order came at a time when the novel coronavirus claimed 16 lives and infected over 600 people across India.

The announcement regarding ban of sale and distribution of the drug was made by the government earlier but it issued an official Gazette notification on Thursday signalling that hydroxychloroquine -- an anti-Malaria drug -- will work as a medicine for treating coronavirus infected patients as well.

Recently, the national task force for COVID-19 constituted by Indian Council for Medical Research (ICMR) has recommended hydroxy-chloroquine as a preventive medication.
