T-Junction 'crisis' looms near end of Thai cave rescue

Mae Sai, Jul 8: Twelve boys and their football coach trapped in a flooded Thai cave will have to squeeze through an extremely narrow tunnel in pitch blackness -- the main "crisis" point that looms near the end of their treacherous escape bid.

Authorities have highlighted the tiny passageway near T-Junction, or Sam Yak in Thai, as the most dangerous element of the journey for the "Wild Boars" team that began Sunday morning, but there are many other potential pitfalls.

The rescue effort is likely to take two to three days to complete, Major General Chalongchai Chaiyakorn, an army commander, told reporters, adding it "depends on other factors like the weather".

Here are some of the challenges that the boys and their coach will face leaving the cave they ventured into on June 23, becoming trapped more than four kilometres (2.4 miles) from the entrance because of monsoon rains.

The boys, aged from 11 and 16, have no diving experience and some can not even swim. They have received training in recent days in preparation for the extraction effort, but they will have to swim using scuba gear through fast-flowing water in darkness, a challenge for even elite divers.

The difficulty of the journey was underscored when a former Thai Navy Seal diver died on Friday after running out of oxygen in the cave.

Thirteen "world class" foreign divers and Thai Navy Seals are involved in the rescue effort. Two divers will escort each of the boys and the coach, aged 25.

The sliver of space is 1.9 kilometres (1.2 miles) from the shelf where the boys have been sheltering above the waters. After energy-sapping efforts navigating jagged tunnels and clambering up or down rock walls for this distance, they will confront Sam Yak.

"The biggest crisis spot for diving is on the left from the T-Junction," said Narongsak Osottanakorn, the rescue mission chief, in a briefing on July 2.

"There is a tunnel that has a passageway going up and coming down narrowly and you have to turn a bit and it's very small."

After that though, the tunnels widen, the waters subside, and walking is even possible, according to authorities, with the rest of the journey expected to be relatively safe as they will have reached a forward operating base inside the cave.

The journey will be a long one. The rescue mission chief, Narongsak Osottanakorn, told reporters on Sunday that the first boy was not expected to emerge until 9pm (1400 GMT) on Sunday. This tallies with previous estimates from officials that it would take the divers five hours to reach the ledge where the team is trapped, and six hours for the journey out.

The boys were found dishevelled and weak nine days after they ventured in. Although they have been receiving food and medicine since then, their lack of strength could be a crucial factor in determining their fate.

The water in the cave is muddy and unclear, with one diver comparing it to a cafe latte. The labyrinth has no outside light. The boys will be helped through the darkness by guiding rope, torches and the escorts.

Nevertheless, the poor visibility is one of the factors raising concerns about the boys -- already traumatised after spending so long in the cave and having to swim underwater -- potentially panicking.

"The mental side of this has to be one of the top considerations," Andrew Watson, an experienced rescuer of mineworkers, previously told AFP.

"Just one individual panicking can cause a problem," he said.

The operation was launched after several days of relatively mild weather, as more than 100 million litres of water were pumped out of the cave.

Kobchai Boonyaorana, deputy director-general of the Disaster Prevention and Mitigation Department of the Interior Ministry, told reporters Sunday that the water level in the cave had continued to recede, and that rainfall was less than expected.

But weather forecasters warned heavy rain was on its way, which could flood the area completely. They said there was a 60-per cent chance of moderate to heavy rain on Sunday afternoon, and that heavier rain would continue from Monday to Thursday.

Washington, May 18: US President Donald Trump on Sunday called his predecessor Barak Obama a ‘grossly incompetent president’.

The Trump’s reaction came after Obama on Saturday criticised the US authorities' response to the coronavirus outbreak.

“He (Obama) was an incompetent president. That’s all I can say. Grossly incompetent,” Trump told reporters at the White House on his arrival from Camp David.

Trump was responding to a question on the virtual commencement address by Obama a day earlier.

In his address to college graduates, Obama had said that the COVID-19 pandemic has exposed the American leadership.

“More than anything, this pandemic has fully, finally torn back the curtain on the idea that so many of the folks in charge know what they’re doing,” Obama said without naming officials.

“A lot of them aren’t even pretending to be in charge,” he added.

There was no immediate response from the office of the former president on the remarks made by Trump.

Beijing, Feb 3: The first batch of patients arrived on Monday at a specialised hospital built in just 10 days as part of China's intensive efforts to fight a new virus.

Huoshenshan Hospital and a second facility with 1,500 beds that's due to open this week were built by construction crews who are working around the clock in Wuhan, the city in central China where the outbreak was first detected in December.

The Wuhan treatment centres mark the second time Chinese leaders have responded to a new disease by building specialised hospitals almost overnight. As severe acute respiratory syndrome, or SARS, spread in 2003, a facility in Beijing for patients with that viral disease was constructed in a week.

The first batch of patients arrived at the Huoshenshan Hospital at 10 am on Monday, according to state media. The reports gave no details of the patients' identities or conditions.

The ruling Communist Party's military wing, the People's Liberation Army, sent 1,400 doctors, nurses and other personnel to staff the Wuhan hospital, the official Xinhua News Agency said. The government said earlier some have experience fighting SARS and other outbreaks.

Authorities have cut most road, rail and air access to Wuhan and surrounding cities, isolating some 50 million people, in efforts to contain the viral outbreak that has sickened more than 17,000 and killed more than 360 people.

The Huoshenshan Hospital was built by a 7,000-member crew of carpenters, plumbers, electricians and other specialists, according to the Xinhua News Agency.               Photos in state media showed workers in winter clothing, safety helmets and the surgical-style masks worn by millions of Chinese in an attempt to avoid contracting the virus.

About half of the two-storey, 600,000-square-foot building is isolation wards, according to the government newspaper Yangtze Daily. It has 30 intensive care units.

Doctors can talk with outside experts over a video system that links them to Beijing's PLA General Hospital, according to the Yangtze Daily. It said the system was installed in less than 12 hours by a 20-member "commando team" from Wuhan Telecom Ltd.

The building has specialised ventilation systems and double-sided cabinets that connect patient rooms to hallways and allow hospital staff to deliver supplies without entering the rooms.

The hospital received a donation of "medical robots" from a Chinese company for use in delivering medicines and carrying test samples, according to the Shanghai newspaper The Paper.

In other cities, the government has designated hospitals to handle cases of the new virus.

In Beijing, the Xiaotangshan Hospital built in 2003 for SARS is being renovated by construction workers. The government has yet to say whether it might be used for patients with the new disease.

Melbourne, Apr 10: Scientists have identified six drug candidates from more than 10,000 compounds that may help treat COVID-19.

The research, published in the journal Nature, tested the efficacy of approved drugs, drug candidates in clinical trials and other compounds.

"Currently there are no targeted therapeutics or effective treatment options for COVID-19," said Professor Luke Guddat from the University of Queensland in Australia.

"In order to rapidly discover lead compounds for clinical use, we initiated a programme of high-throughput drug screening, both in laboratories and also using the latest computer software to predict how different drugs bind to the virus," Guddat said.

The project targeted the main COVID-19 virus enzyme, known as the main protease or Mpro, which plays a pivotal role in mediating viral replication, the researchers said.

This makes it an attractive drug target for this virus, and as people don't naturally have this enzyme, compounds that target it are likely to have low toxicity, they said.

"We add the drugs directly to the enzyme or to cell cultures growing the virus and assess how much of each compound is required to stop the enzyme from working or to kill the virus. If the amount is small, then we have a promising compound for further studies," said Guddat.

After assaying thousands of drugs, researchers found of the six that appear to be effective in inhibiting the enzyme, one is of particular interest.

"We're particularly looking at several leads that have been subjected to clinical trials including for the prevention and treatment of various disorders such as cardiovascular diseases, arthritis, stroke, atherosclerosis and cancer," Guddat said.

Researchers said compounds that are already along the pipeline to drug discovery are preferred, as they can be further tested as antivirals at an accelerated rate compared to new drug leads that would have to go through this process from scratch.

"With continued and up-scaled efforts we are optimistic that new candidates can enter the COVID-19 drug discovery pipeline in the near future," Guddat said.
