In Vitro Fertilisations Babies Born To Women Over 40 May Have Lesser Birth Defects: Study

Melbourne, Oct 17: Babies born to women aged over 40 from assisted reproduction have fewer birth defects compared with those from women who conceive naturally at that age, a new study has found.

pregnantThis is contrary to widespread belief that the greater risk of birth defects after assisted conception is due to the frequent use of these services by older women.

According to researchers from the University of Adelaide in Australia, this may point to the presence of more favourable biological conditions in IVF (in vitro fertilisations) specific to pregnancies in older women - but they are currently working to determine the exact cause.

The research is based on data of all live births recorded in Australia from 1986-2002, including more than 301,000 naturally conceived births, as well as 2,200 births from IVF and almost 1400 from ICSI (intracytoplasmic sperm injection).

The average prevalence of a birth defect was 5.7 per cent among naturally conceived births, 7.1 per cent for the IVF births, and 9.9 per cent for the ICSI births, across all age groups.

In births from assisted reproduction, the prevalence of birth defects ranged from 11.3 per cent at its highest for women less than age 30 using ICSI, down to 3.6 per cent for women aged 40 and older using IVF.

For natural conceptions, the corresponding prevalence across age groups was 5.6 per cent in young women, increasing to 8.2 per cent in women aged 40 above.

"There is something quite remarkable occurring with women over the age of 40 who use assisted reproduction," said lead author Michael Davies, professor from University of Adelaide.

"We know from our previous studies that women who undergo assisted reproduction have an increased rate of birth defects compared to women who conceive naturally," said Davies.

"We also know that among women who conceive naturally, the rate of birth defects increases exponentially from age 35 onwards. Therefore, it was widely assumed, but untested, that maternal age would be a key factor in birth defects from assisted reproduction," Davies added.

"However, our findings challenge that assertion. They show that infertile women aged 40 and over who used assisted reproduction had less than half the rate of birth defects of fertile women of the same age, while younger women appear to be at an elevated risk," he said.

"For women treated with IVF and ICSI combined, the greatest age-related risk of birth defects was among the young women, at or around 29 years of age," he said.

"With a prevalence of 9.4 per cent their risk was more than double the rate of 3.6 per cent observed for the patients aged over 40, and significantly higher than for fertile women of the same age, at 5.6 per cent," Davies said.

"There is some aspect of IVF treatment in particular that could be helping older women to redress the maternal age issues we see among natural conception, where we observe a transition at around the age of 35 years toward a steadily increasing risk of birth defects," he said.

The study appears in BJOG: An International Journal of Obstetrics and Gynaecology.

High-protein diets may help people lose weight and build muscle, but there is a downside to it --a greater heart attack risk. Researchers now report that high-protein diets boost artery-clogging plaque.

The research in mice showed that high-protein diets spur unstable plaque -- the kind most prone to rupturing and causing blocked arteries.

More plaque buildup in the arteries, particularly if it's unstable, increases the risk of heart attack.

"There are clear weight-loss benefits to high-protein diets, which has boosted their popularity in recent years," said senior author Babak Razani, associate professor at Washington University School of Medicine in St. Louis, Missouri.

"But animal studies and some large epidemiological studies in people have linked high dietary protein to cardiovascular problems. We decided to take a look at whether there is truly a causal link between high dietary protein and poorer cardiovascular health," Razani added.

The researchers studied mice who were fed a high-fat diet to deliberately induce atherosclerosis, or plaque buildup in the arteries.

Some of the mice received a high-fat diet that was also high in protein. And others were fed a high-fat, low-protein diet for comparison.

The mice on the high-fat, high-protein diet developed worse atherosclerosis -- about 30 per cent more plaque in the arteries -- than mice on the high-fat, normal-protein diet, despite the fact that the mice eating more protein did not gain weight, unlike the mice on the high-fat, normal-protein diet.

"A couple of a scoop of protein powder in a milkshake or smoothie adds something like 40 grams of protein -- almost equivalent to the daily recommended intake," Razani said.

"To see if protein has an effect on cardiovascular health, we tripled the amount of protein that the mice receive in the high-fat, high-protein diet -- keeping the fat constant. Protein went from 15 per cent to 46 per cent of calories for these mice".

Plaque contains a mix of fat, cholesterol, calcium deposits and dead cells. Past work by Razani's team and other groups has shown that immune cells called macrophages work to clean up plaque in the arteries.

But the environment inside plaque can overwhelm these cells, and when such cells die, they make the problem worse, contributing to plaque buildup and increasing plaque complexity.

"In mice on the high-protein diet, their plaques were a macrophage graveyard," Razani informed.

To understand how high dietary protein might increase plaque complexity, Razani and his colleagues also studied the path protein takes after it has been digested -- broken down into its original building blocks, called amino acids.

"This study is not the first to show a telltale increase in plaque with high-protein diets, but it offers a deeper understanding of the impact of high protein with the detailed analysis of the plaques," said Razani.

"This work not only defines the critical processes underlying the cardiovascular risks of dietary protein but also lays the groundwork for targeting these pathways in treating heart disease," he added.

While coughing, fever and difficulty in breathing are common symptoms of COVID-19, a new case study has found that pink eye is also a reason to be tested for the disease.

The study, published in the Canadian Journal of Ophthalmology, determined that conjunctivitis and keratoconjunctivitis can also be primary symptoms of COVID-19.

The researchers noted that in March, a 29-year-old woman arrived at the Royal Alexandra Hospital's Eye Institute of Alberta with a severe case of conjunctivitis and minimal respiratory symptoms.

After the patient had undergone several days of treatment with little improvement -- and after it had been determined that the woman had recently returned home from Asia -- a resident ordered a COVID-19 test.

The test came back positive, according to the researchers.

"What is interesting in this case, and perhaps very different to how it had been recognised at that specific time, was that the main presentation of the illness was not a respiratory symptom. It was the eye," said Carlos Solarte, an assistant professor at the University of Alberta in Canada.

"There was no fever and no cough, so we weren't led to suspect COVID-19 at the beginning. We didn't know it could present primarily with the eye and not with the lungs," Solarte said.

Academic studies at the outset of the pandemic identified conjunctivitis as a secondary symptoms in about 10 to 15 per cent of COVID-19 cases, he said.

Since then, scientists have gained greater knowledge of how the virus can transmit through and affect the body's mucous membrane system, of which the conjunctiva -- the clear, thin membrane that covers the front surface of the eye -- is an extension.

While the finding provides important new health information for the public, it also makes eye exams more complicated for ophthalmologists and staff, the researchers noted.

"The patient in this case eventually recovered well without any issues. But several of the residents and staff who were in close contact with the patient had to be under quarantine," said Solarte.

"Fortunately, none who were involved in her care also tested positive," he said.

Patients coming into an eye clinic with conjunctivitis and keratoconjunctivitis are now treated as potential cases of COVID-19 and extra precautions are taken by staff, according to the researchers.
