Faulty data misled pilot in 2009 Air France plane crash: Report

Le Bourget (France), July 6: A combination of faulty sensors and mistakes by inadequately trained pilots caused an Air France jet to plunge into the Atlantic Ocean in 2009, killing all 228 people aboard in the airline's deadliest ever crash, French investigators said on Thursday.

Investigators are urging better instruction for pilots on flying manually at high altitudes and stricter plane certification rules as a result of a three-year investigation into what happened to Flight 447.

Airbus, manufacturer of the A330 plane, said in a statement that it is working to improve speed sensors known as pitot tubes and making other efforts to avoid future such accidents. Air France stressed the equipment troubles and insisted the pilots "acted in line with the information provided by the cockpit instruments and systems. .... The reading of the various data did not enable them to apply the appropriate action."


But the Bureau for Investigations and Analysis' findings raised broader concerns about training for pilots worldwide flying high-tech planes when confronted with a high-altitude crisis.

The report also could have legal implications: A separate French judicial investigation is still under way, and Air France and Airbus have been handed preliminary manslaughter charges.

The bureau's analysis lists a combination of "human and technical factors" behind the crash. The plane flying from Rio de Janeiro to Paris slammed into the sea during a nighttime thunderstorm on June 1, 2009.

Some families of victims felt investigators didn't focus hard enough on the equipment problems, saying the two pilots at the controls were doing what they could while faced with a barrage of inaccurate information.

Ice crystals that blocked the pitot tubes were the "unleashing event" that set off the plane's troubles, chief investigator Alain Bouillard said. The plane's autopilot shut off and the co-pilots had to fly manually, while a succession of alarms were going off. The captain was on a rest break.

In one fatal decision, the report says, one of the co-pilots nosed the Airbus A330 upward during a stall - instead of downward, as he should have - because of false data from sensors about the plane's position. Mr Bouillard said that was an "important element" of the cause of the crash.

He said the two pilots at the controls never understood that the plane was in a stall. He said only a well-experienced crew with a clear understanding of the situation could have stabilized the plane in those conditions.

"In this case, the crew was in a state of near-total loss of control," Mr Bouillard said.

Robert Soulas, who lost his daughter and son-in-law in the crash, says investigators said the flight director system indicated the "erroneous information" that the plane was diving downward, "and therefore to compensate, the pilot had a tendency to pull on the throttle to make it rise up."

However, the plane was in a stall instead. A basic maneuver for stall recovery, which pilots are taught at the outset of their flight training, is to push the yoke forward and apply full throttle to lower the nose of the plane and build up speed. But because the pilot thought the plane was diving, he nosed up.

Some families of people who died in the crash showed sympathy toward the pilots, saying they were dealing with bad equipment in an exceptionally challenging situation.

Mr Soulas noted that manufacturers had known for years about problems with the plane's speed sensors freezing over, but didn't order the faulty models systematically replaced until after the crash.

Pilot Gerard Arnoux said, "A normal pilot on a normal airliner follows" the signals on the flight director system, which tells them to go left, right, up or down.

Central to this accident is the fact that when the automation failed, the pilots were presented with conflicting information which was obviously incorrect, said William Voss, president of the Flight Safety Foundation in Alexandria, Virginia. But they were unable to look through this and understand what the aircraft was actually doing.

"Pilots a generation ago would have done that and understand what was going on, but (the AF447 pilots) were so conditioned to rely on the automation that they were unable to do this," he said.

"This is a problem not just limited to Air France or Airbus," Mr Voss said. "It's a problem we're seeing around the world because pilots are being conditioned to treat automated processed data as truth, and not compare it with the raw information that lies underneath."

The final report included a study of the plane's black box flight recorders, uncovered in a costly and extraordinarily complex search in the ocean depths.

Lais Seba, the mother of 31-year-old victim Luciana Clarkson Seba, said "it's going to be forever difficult" for survivors to deal with the loss of their loved-ones.

"We are surviving," she said. "We live one day at a time, with lots of pain, and always missing her."

Washington, Apr 28: After nearly three weeks in an intensive care unit in Los Angeles, doctors treating 41-year-old Broadway actor Nick Cordero for COVID-19 were forced to amputate his right leg.

The flow of blood had been impeded by a blood clot: yet another dangerous complication of the disease that has been bubbling up in frontline reports from China, Europe and the United States.

To be sure, so-called "thrombotic events" occur for a variety of reasons among intensive care patients, but the rates among COVID-19 patients are far higher than would be otherwise expected.

"I have had 40-year-olds in my ICU who have clots in their fingers that look like they'll lose the finger, but there's no other reason to lose the finger than the virus," Shari Brosnahan, a critical care doctor at NYU Langone said.

One of these patients is suffering from a lack of blood flow to both feet and both hands, and she predicts an amputation may be necessary, or the blood vessels may get so damaged that an extremity could drop off by itself.

Blood clots aren't just dangerous for our limbs, but can make their way to the lungs, heart or brain, where they may cause lethal pulmonary embolisms, heart attacks, and strokes.

A recent paper from the Netherlands in the journal Thrombosis Research found that 31 percent of 184 patients suffered thrombotic complications, a figure that the researchers called "remarkably high" -- even if extreme consequences like amputation are rare.

Behnood Bikdeli, a doctor at New York-Presbyterian Hospital, assembled an international consortium of experts to study the issue. Their findings were published in the Journal of The American College of Cardiology.

The experts found the risks were so great that COVID-19 patients "may need to receive blood thinners, preventively, prophylactically," even before imaging tests are ordered, said Bikdeli.

What exactly is causing it? The reasons aren't fully understood, but he offered several possible explanations.

People with severe forms of COVID-19 often have underlying medical conditions like heart or lung disease -- which are themselves linked to higher rates of clotting.

Next, being in intensive care makes a person likelier to develop a clot because they are staying still for so long. That's why for example people are encouraged to stretch and move around on long haul flights.

It's also now clear the COVID-19 illness is associated with an abnormal immune reaction called "cytokine storm" -- and some research has indicated this too is linked to higher rates of clotting.

There could also be something about the virus itself that is causing coagulation, which has some precedent in other viral illnesses.

A paper in the journal The Lancet last week showed that the virus can infect the inner cell layer of organs and of blood vessels, called the endothelium. This, in theory, could interfere with the clotting process.

According to Brosnahan, while thinners like Heparin are effective in some patients, they don't work for all patients because the clots are at times too small.

"There are too many microclots," she said. "We're not sure exactly where they are."

Autopsies have in fact shown some people's lungs filled with hundreds of microclots.

The arrival of a new mystery however helps solve a slightly older one.

Cecilia Mirant-Borde, an intensive care doctor at a military veterans hospital in Manhattan, told AFP that lungs filled with microclots helped explain why ventilators work poorly for patients with low blood oxygen.

Earlier in the pandemic doctors were treating these patients according to protocols developed for acute respiratory distress syndrome, sometimes known as "wet lung."

But in some cases, "it's not because the lungs are occupied with water" -- rather, it's that the microclotting is blocking circulation and blood is leaving the lungs with less oxygen than it should.

It has just been a little under five months since the virus emerged in Wuhan, China, and researchers are learning more about its impact every day.

"While we react surprised, we shouldn't be as surprised as we were. Viruses tend to do weird things," said Brosnahan.

While the dizzying array of complications may seem daunting, "it's possible there'll be one or a couple of unifying mechanisms that describe how this damage happens," she said.

"It's possible it's all the same thing, and that there'll be the same solution."

New York, May 8: An Indian-American father and daughter, both doctors in New Jersey, have died due to the COVID-19, with Governor Phil Murphy describing their demise as “particularly tough" and hailed them for dedicating their lives for others.

Satyender Dev Khanna, 78, was a surgeon who served both on staff and as the head of the surgical departments for multiple hospitals across New Jersey for decades.

Priya Khanna, 43, was a double board-certified in both internal medicine and nephrology. She was Chief of Residents at Union Hospital, now part of RWJ Barnabas Health.

"Dr Satyender Dev Khanna and Dr Priya Khanna were father and daughter. They both dedicated their lives to helping others. This is a family dedicated to health and medicine. Our words cannot amply express our condolences," New Jersey Governor Murphy tweeted on Thursday.

“Both dedicated their lives to helping others and we lost both of them to COVID-19,” Murphy said during a press conference on Thursday, saying their demise is a "particularly tough one.”

Satyender passed away at the Clara Maass Medical Center where he had worked for more than 35 years.

Murphy described him as a "pioneering doctor” who was one of the first surgeons to perform laparoscopic surgery in the state. He is being remembered by colleagues as a “gentle and caring physician."

“And for a doctor, I'm not one, but I would bet, I don't think there could be a more fitting way to be remembered, or a nurse or a healthcare worker of any kind,” Murphy said, adding that the doctor had a passion for bicycling, and he often found peace from the hustle of the hospital in biking along the Jersey Shore.

Priya did all of her medical training in New Jersey and then did her fellowship in nephrology in South Jersey with the Cooper Health System. Like her father, she too worked at Clara Maass, where she died.

She was also Medical Director at two dialysis centres in Essex County and “took pride” in teaching the next generation of doctors, Murphy said, adding that the ICU physician who cared for Priya Khanna was trained and taught by her as well.

“Priya will be remembered as a caring and selfless person who put others first. And even while in the hospital, fighting her own battle, she continued to check up on her mom and dad and her family,” Murphy said.

“This is a family, by the way, dedicated to health and medicine,” he said.

The governor spoke with Satyender's wife Komlish Khanna, who is a paediatrician. The couple has two more daughters - Sugandha Khanna, an emergency medicine physician and Anisha Khanna, a paediatrician.

“Unbelievable. Our words cannot amply express our condolences nor, I am sure, can they express the pain that the Khanna family is feeling. But I hope that the fact that our entire state mourns with them is some small comfort. And we mourn everyone we have lost. We commit in their memory to saving as many lives as we can,” Murphy said.

London, Jun 19: Malala Yousafzai, the youngest Nobel Peace Prize winner who once took a bullet for campaigning for girls' education in Pakistan, was over the moon on Friday after completing her degree in Philosophy, Politics and Economics at Britain's prestigious Oxford University.

Malala, 22, who attended Oxford's Lady Margaret Hall college, took to Twitter to share two pictures that show her celebrating the milestone with her family.

"Hard to express my joy and gratitude right now as I completed my Philosophy, Politics and Economics degree at Oxford," she said in the tweet, accompanied by two pictures - one showing her sitting with her family in front of a cake that says: 'Happy Graduation Malala', and the other in which she is covered with cake smiling for the camera.

In the tweet, the famed human rights activist also revealed her plans for the immediate future - Netflix, reading and sleeping.

"I don't know what's ahead. For now, it will be Netflix, reading and sleep," she wrote.

Malala was shot in the head by the Taliban militants in December 2012 for campaigning for female education in the Swat Valley in northeastern Pakistan.

Severely wounded, she was airlifted from one military hospital in Pakistan to another and later flown to the UK for treatment.

After the attack, the Taliban released a statement saying that they would target Malala again if she survived.

At the age of 17, Malala became the youngest recipient of the Nobel Peace Prize for her education advocacy in 2014 when she shared the coveted honour with India's social activist Kailash Satyarthi.

Unable to return to Pakistan after her recovery, she moved to Britain, setting up the Malala Fund and supporting local education advocacy groups with a focus on Pakistan, Nigeria, Jordan, Syria and Kenya.

The Taliban, who are against girls' education, have destroyed many schools in Pakistan.
