Better to act young than to look young

Young_than_to_look_youngJane E Brody, Oct 7: At a party I recently attended, a woman in her 60s proudly announced that her periodic facial treatments “have made me look 10 years younger.” A man of similar vintage said he was considering “facial tucks” to raise his sagging jowls.

Some days it seems everyone I meet is afraid of getting old - or at least of looking as old as they are. Occasionally, I see women who have had so many face lifts that they can barely move their lips when they talk, let alone smile.

Business is booming in the anti-aging market. Plastic surgeons who specialise in lifts, tucks and fillers barely noticed the recent recession. Cosmetics with anti-ageing properties fly off the shelf, and new concoctions appear almost weekly.

I admit to supporting the multi-billion dollar skin care industry with my long use of night creams, as well as a slew of daytime lotions that purport to “smooth out” aging skin while protecting it with sunscreen. I also colour my hair, which in its natural state is now about 80 per cent gray.

But I draw the line at injectable fillers and muscle relaxants, face lifts and tummy tucks. I’ll do everything I can to stay out of an operating room. My anti-aging measures, if and when they are needed, will be limited to cataract removal, a hearing aid and glasses for driving.

I wear clothes that suit my personality and activities, not necessarily my age. Shorts and tank tops anchor my summer wardrobe. And I wear colours near my face that are right for my skin tone, avoiding those (like yellow) that are unflattering. Still, cosmetic fixes go only so far to counter the effects of time.

True youthfulness - or, I should say, appearing younger than your age - is much more than skin-deep. Research has shown that youthfulness must come from within.

In one study, a team at Case Western Reserve University in Cleveland analyzed the facial photographs of 186 pairs of identical twins, determining which sibling looked older and why. Factors that contributed to looking older included smoking, sun exposure, stress and depression (or the use of antidepressants), the researchers reported. Other studies have linked depression to higher levels of inflammatory markers and oxidative stress, which can accelerate aging.

The team also found that among those younger than 40, a woman with a heavier body looked older than her leaner twin, but in subjects older than 40, a higher body mass index was associated with a more youthful appearance. Fat fills out wrinkles and makes the face - and presumably other parts of the body - look younger. But there’s a limit to its benefits: obesity is associated with more rapid biological aging.

Another study published last year by researchers at Brigham and Women’s Hospital in Boston found a biological link between stress and accelerated aging. Dr. Olivia I. Okereke and colleagues examined the chromosomes of 5,243 women ages 42 to 69 participating in the Nurses’ Health Study. They found that women with phobic anxiety - irrational fears of anything from social situations to spiders - had shorter telomeres on their chromosomes.

Telomeres are DNA-protein complexes on the ends of chromosomes that are considered biological markers of aging. Shortened telomeres are thought to underlie many of the adverse health effects of aging, and perhaps even to contribute to looking older than one’s years.

Lifestyle changes

A pilot study published online by The Lancet Oncology last month underscored the benefits of limiting stress. Dr. Dean Ornish and colleagues at the Preventive Medicine Research Institute and the University of California, San Francisco, tested the effects of lifestyle changes on the length of telomeres.

Ten men were asked to make changes that included adopting a whole-foods plant-based diet, moderate exercise, stress management techniques (like yoga and meditation), and seeking greater intimacy and social support. After five years, changes in the length of their telomeres were compared with those among 25 men who were not asked to make such changes.

In the men who made lifestyle changes, telomere length increased by an average of 10 per cent; the more changes the men made, the greater the increase in length. But telomere length decreased in the control group by an average of 3 per cent.

Still, youthfulness is not just a question of biology. People are perceived to be younger than their years if they smile and laugh a lot (be proud of those laugh lines!) and are generally cheerful and upbeat, the kind of people who smile at strangers and wish them a good day.

I occasionally pass an older couple who walk together three times a week. With their dour expressions and grudging acknowledgment of my “good morning,” I thought they were in their mid-90s. So one day I asked, only to learn that she was 80 and he was 83.

It also helps to pursue personal and professional activities you enjoy as you get older. My role models include the actress Estelle Parsons, who continues to perform on stage (and can do yoga headstands) at 85, and Dame Judi Dench, 78, who stars in movies despite having to learn her lines orally because macular degeneration has caused her vision to deteriorate.

My dear friend Margaret Shryer, 86, starred in a documentary about age discrimination and performed a one-woman show she wrote at a theater festival in Minneapolis this summer. Another friend, the award-winning broadcaster Lucy Jarvis, cane in hand at 96, still travels abroad to make television documentaries.

To my mind, it is far better to act young than to look young. Of course, it helps to be physically fit.

Too often, those who spend liberally to counter the superficial signs of age neglect their bodies below the neck and become physically old before their time. Muscle tissue inevitably declines with age, but much can be done to minimize and even reverse a loss of strength.

Try using free weights or resistance machines. If these don’t appeal, lift cans of soup (better for lifting than consuming) and continue to do chores that require strength, like digging in the garden or carrying groceries.

Washington D.C., May 20: While a dairy-rich diet is helpful in meeting the body's calcium requirement, outcomes of a large international study links eating at least two daily servings of dairy with lower risks of diabetes and high blood pressure.

The dairy-rich diet also proved to lower the cluster of factors that heighten cardiovascular disease risk (metabolic syndrome). The study was published online in journal BMJ Open Diabetes Research & Care.

The observed associations were strongest for full-fat dairy products, the findings indicated.

Previously published research has suggested that higher dairy intake is associated with a lower risk of diabetes, high blood pressure, and metabolic syndrome. But these studies have tended to focus on North America and Europe to the exclusion of other regions of the world.

To see whether these associations might also be found in a broader range of countries, the researchers drew on people taking part in the Prospective Urban Rural Epidemiology (PURE) study.

Participants were all aged between 35 and 70 and came from 21 countries: Argentina; Bangladesh; Brazil; Canada; Chile; China; Colombia; India; Iran; Malaysia; Palestine; Pakistan; Philippines, Poland; South Africa; Saudi Arabia; Sweden; Tanzania; Turkey; United Arab Emirates; and Zimbabwe.

Usual dietary intake over the previous 12 months was assessed by means of Food Frequency Questionnaires. Dairy products included milk, yogurt, yogurt drinks, cheese and dishes prepared with dairy products, and were classified as full or low fat (1-2 percent).

Butter and cream were assessed separately as these are not commonly eaten in some of the countries studied.

Information on personal medical history, use of prescription medicines, educational attainment, smoking and measurements of weight, height, waist circumference, blood pressure and fasting blood glucose were also collected.

Data on all five components of the metabolic syndrome were available for nearly 113,000 people: blood pressure above 130/85 mm Hg; waist circumference above 80 cm; low levels of (beneficial) high-density cholesterol (less than 1-1.3 mmol/l); blood fats (triglycerides) of more than 1.7 mmol/dl; and fasting blood glucose of 5.5 mmol/l or more.

Average daily total dairy consumption was 179 g, with full-fat accounting for around double the amount of low fat: 124.5+ vs 65 g.

Some 46, 667 people had metabolic syndrome--defined as having at least 3 of the 5 components.

Total dairy and full-fat dairy, but not low-fat dairy, was associated with a lower prevalence of most components of metabolic syndrome, with the size of the association greatest in those countries with normally low dairy intakes.

At least 2 servings a day of total dairy were associated with a 24 percent lower risk of metabolic syndrome, rising to 28 percent for full-fat dairy alone, compared with no daily dairy intake.

The health of nearly 190,000 participants was tracked for an average of nine years, during which time 13,640 people developed high blood pressure and 5351 developed diabetes.

At least 2 servings a day of total dairy was associated with a 11-12 percent lower risk of both conditions, rising to a 13-14 percent lower risk for 3 daily servings. The associations were stronger for full fat than they were for low-fat dairy.

This is an observational study, and as such can't establish the cause. Food frequency questionnaires are also subject to recall, and changes in metabolic syndrome weren't measured over time, all of which may have influenced the findings.

Nevertheless, the researchers suggest: "If our findings are confirmed in sufficiently large and long term trials, then increasing dairy consumption may represent a feasible and low-cost approach to reducing [metabolic syndrome], hypertension, diabetes, and ultimately cardiovascular disease events worldwide."

Europe, Jan 11: Researchers have revealed the people who drink tea at least three times a week have healthy years of life and longer life expectancy.

The research was published in the European Journal of Preventive Cardiology, a journal of the European Society of Cardiology (ESC).

Dr Xinyan Wang, who is the author of the study, said: "Habitual tea consumption is associated with lower risks of cardiovascular disease and all-cause death. The favourable health effects are the most robust for green tea and for long-term habitual tea drinkers."
The analysis that was conducted included about 100,902 participants of the China-PAR project2 with no history of heart attack, stroke, or cancer.

Participants were classified into two groups: Habitual tea drinkers and never or non-habitual tea drinkers and followed-up for a median of 7.3 years.

The analyses estimated that 50-year-old habitual tea drinkers would develop coronary heart disease and stroke 1.41 years later and live 1.26 years longer than those who never or seldom drank tea. Compared with never or non-habitual tea drinkers, the habitual tea consumers had a 20 per cent lower risk of incident heart disease and stroke, 22 per cent lower risk of fatal heart disease and stroke, and 15 per cent decreased risk of all-cause death.

The potential influence of changes in tea drinking behaviour was suspected in a subset of 14,081 participants with assessments at two-time points. The average duration between the two surveys was 8.2 years, and the median follow-up after the second survey was 5.3 years.

Habitual tea drinkers who maintained their habit in both surveys had a 39 per cent lower risk of incident heart disease and stroke, 56 per cent lower risk of fatal heart disease and stroke, and 29 per cent decreased risk of all-cause death compared to consistent never or non-habitual tea drinkers.

Senior author Dr Dongfeng Gu said: "The protective effects of tea were most pronounced among the consistent habitual tea drinking group. Mechanism studies have suggested that the main bioactive compounds in tea, namely polyphenols, are not stored in the body long-term. Thus, frequent tea intake over an extended period may be necessary for the cardioprotective effect."

In a subanalysis by type of tea, drinking green tea was linked with approximately 25 per cent lower risks for incident heart disease and stroke, fatal heart disease and stroke, and all-cause death. However, no significant associations were observed for black tea.
Dr Gu noted that a preference for green tea is unique to East Asia.

Two factors may be at play. First, green tea is a rich source of polyphenols which protect against cardiovascular disease and its risk factors including high blood pressure and dyslipidaemia. Black tea is fully fermented and during this process, polyphenols are oxidised into pigments and may lose their antioxidant effects. Second, black tea is often served with milk, which previous research has shown may counteract the favourable health effects of tea on vascular function.

Gender-specific analyses showed that the protective effects of habitual tea consumption were pronounced and robust across different outcomes for men, but only modest for women. Dr Wang said: "One reason might be that 48 per cent of men were habitual tea consumers compared to just 20 per cent of women. Secondly, women had a much lower incidence of, and mortality from, heart disease and stroke. These differences made it more likely to find statistically significant results among men."

She said: "The China-PAR project is ongoing, and with more person-years of follow-up among women the associations may become more pronounced."

In conclusion, the authors have found that randomised trials are required to validate the results and to illustrate nutritional guidelines and advice for lifestyle.

New York, May 19: Cigarette smoke spurs the lungs to make more of the receptor protein which the novel coronavirus uses to enter human cells, according to a study which suggests that quitting smoking might reduce the risk of a severe coronavirus infection.

The findings, published in the journal Developmental Cell, may explain why smokers appear to be particularly vulnerable to severe COVID-19 disease.

"Our results provide a clue as to why smokers who develop COVID-19 tend to have poor clinical outcomes," said study senior author Jason Sheltzer, a cancer geneticist at Cold Spring Harbor Laboratory in the US.

"We found that smoking caused a significant increase in the expression of ACE2, the protein that SARS-CoV-2 uses to enter human cells," Sheltzer said.

According to the scientists, quitting smoking might reduce the risk of a severe coronavirus infection.

They said most individuals infected with the virus suffer only mild illness, if they experience any at all.

However, some require intensive care when the sometimes-fatal virus attacks, the researchers said.

In particular, they said three groups have been significantly more likely than others to develop severe illness -- men, the elderly, and smokers.

Turning to previously published data for possible explanations for these disparities, the scientists assessed if vulnerable groups share some key features related to the human proteins that the coronavirus relies on for infection.

First, they said, they focused on comparing gene activity in the lungs across different ages, between the sexes, and between smokers and nonsmokers.

The scientists said both mice that had been exposed to smoke in a laboratory, and humans who were current smokers had significant upregulation of ACE2.

According to Sheltzer, smokers produced 30-55 per cent more ACE2 than their non-smoking counterparts.

While the researchers found no evidence that age or sex impacts ACE2 levels in the lungs, they said the influence of smoke exposure was surprisingly strong.

However, they said, the change seemed to be temporary.

According to the data, the level of the receptors ACE2 in the lungs of people who had quit smoking was similar to that of non-smokers.

The study noted that the most prolific producers of ACE2 in the airways are mucus-producing cells called goblet cells.

Smoking is known to increase the prevalence of such cells, the scientists said.

"Goblet cells produce mucous to protect the respiratory tract from inhaled irritants. Thus, the increased expression of ACE2 in smokers' lungs could be a byproduct of smoking-induced secretory cell hyperplasia," Sheltzer explained.

However, Sheltzer said other studies on the effects of cigarette smoke have shown mixed results.

"Cigarette smoke contains hundreds of different chemicals. It's possible that certain ingredients like nicotine have a different effect than whole smoke does," he said.

The researchers cautioned that the actual ACE2 protein may be regulated in ways not addressed in the current study.

"One could imagine that having more cells that express ACE2 could make it easier for SARS-CoV-2 to spread in someone's lungs, but there is still a lot more we need to explore," Sheltzer said.
