Private hospitals will have to display treatment costs

New Delhi, October 8: In a major move aimed at checking unnecessary medical tests and procedures, the health ministry on Sunday said it was working on a plan to make it mandatory for all private hospitals to declare and display the treatment costs of different diseases.

Jagdish Prasad, director general of health services (DGHS), told TOI that the ministry's plan would check overbilling and bring in transparency in the healthcare sector.

"Our aim is to notify standard costs for various medical procedures such as angioplasty, coronary bypass surgery etc. We also plan to categorize hospitals based on quality of services offered and develop standard treatment costs for each category," DGHS Prasad said.

He said suggestions are being sought from all stakeholders, including the corporate-run hospitals, on how the new system can be rolled out.

"We will initially ask each hospital to give their treatment charges, which would help us slot these hospitals into categories," Prasad said. A standard guideline for treating conditions like heart attack, stroke, diabetes, osteoporosis and cancer, is also on the anvil, the senior ministry official said.

Unwarranted procedures add to cost

Prasad said the guidelines will be introduced under the Clinical Establishment (Registration and Regulation) Act 2010, which was notified recently.

The planned guidelines would also give a boost to evidence-based approach to treatment, said Dr Kameshwar Prasad, head of the clinical epidemiology unit of AIIMS. "Unwarranted procedures and diagnostic tests put extra financial burden on the healthcare system and cause increased morbidity and mortality. For example, a person suffering from muscular chest pain does not require angiography. Still, many hospitals routinely put such patients through this procedure," the doctor said.

Experts cite studies showing rampant use of unnecessary procedures in India. For instance, hormone replacement therapy is carried out in post-menopausal women despite there being no evidence that it reduces cardiac events. Similarly, antioxidant vitamins are commonly prescribed for reducing cardiovascular disease though there's no evidence of its efficacy. The Elisa test has till recently been widely used for diagnosis of tuberculosis. It has since been banned due to its unreliability.

The DGHS also said patient treatment cannot be taken as a clinical trial. "Stem cell therapy is being offered for spine injuries by many hospitals and patients are charged for that. This despite the fact that there's no proven effect of the therapy. This can only be stopped through patient awareness and strict monitoring by government agencies," Prasad said.

AIIMS is leading a first-of-its kind global initiative to promote evidence-based healthcare. Said institute director, Dr R C Deka, "Awareness of the latest evidence-based treatments is necessary. However, treatment costs should also be borne in mind by doctors while suggesting procedures for patients."Deka also said overprescription was also leading to immunity against many antibiotics, a problem that was likely to become serious in the future. Standardized procedures should help in curbing this trend, he added.

New Delhi, Jun 23: In an unexpected development, the pump price of diesel is all set to surpass the petrol price in the capital, making it the most expensive transport fuel for the first time in a long time.

Globally, diesel is priced slightly above petrol prices due to the very nature of the product that has a higher cost of production. But in India, due to the lopsided taxation structure, diesel attracts lesser of the tax between the two auto fuels keeping its prices lower than petrol for last several years.

Diesel is currently priced at Rs 79.40 a litre in the Capital, just 36 paise short of petrol price that is being retailed at Rs 79.76 a litre. Going by the trend of price movement in the two products for the last few days where diesel prices have consistently increased by 50-60 paise per litre while the daily increase in petrol prices have fallen to just 20 paise on Tuesday, it is set to surpass petrol prices in next few days.

"Diesel price movement is sharper in international market and if oil companies follow the global price trend, diesel prices will surpass that of petrol later this week. It will be after many years that this would happen and is expected to sustain for some time unless government changes the tax structure of the petroleum products again," said an oil sector expert from one of the big four audit and advisory firms asking not to be named.

Interestingly, even in India the base price of diesel is expensive than petrol. According to the Indian Oil Corporation (IOC), while the base price of petrol in Delhi currently comes to Rs 22.11 per litre, the same for diesel is higher at Rs 22.93 per litre (effective from June 16, 2020). This has been the case for a long time, but retail price of petrol can be higher than diesel due to central and state taxes.

What has now brought diesel prices to a whisker of petrol prices in the capital is the Delhi government's decision early May to increase the Value Added Tax on diesel from 16.75 per cent to 30 per cent and on petrol from 27 per cent to 30 per cent. This increased the retail price of diesel and petrol in Delhi by Rs 7.10 and Rs 1.67 a litre respectively. With Central taxes on the two products already reaching identical levels, the Delhi governments move hastened price parity between petrol and diesel.

Currently, the Central excise on petrol is Rs 32.98 a litre while that on diesel it is Rs 31.83 a litre. The VAT on petrol in Delhi is Rs 17.71 a litre and that on diesel is Rs 17.60 a litre.

While the movement of retail pricing is being seen with a sigh of relief by vehicle owners whose cars run on petrol, those buying the relatively expensive diesel cars are now repenting on their decision. The development is also being seen with caution by automobile companies who have spent millions to ramp up their facilities for diesel run vehicles. The expectation is that demand for such cars will now fall, causing more damage to companies where sales are already impacted due to persistent economic slowdown and now the spread of COVID-19 pandemic.

"The pricing development would push automobile companies to strategies being followed by companies in the western markets where diesel run cars are not sold on fuel pricing differential, but on overall make and quality that puts them ahead of petrol run cars," the expert quoted earlier.

Yes, but for commercial vehicle sector the rising price of diesel had not been welcomed. In fact, the commercial transport sector had time an again threatened strike against the move to raise fuel prices.

With petrol and diesel retail prices closing, the case for adultering fuel has also gone down much to the relief of vehicle owners.

New Delhi, Jul 6: India's COVID-19 tally neared the 7 lakh mark with 6,97,413 cases after 24,248 new cases were reported in the last 24 hours, said the Union Ministry of Health and Family Welfare.

As per the Health Ministry, there are 2,53,287 active cases in the country while 4,24,432 patients have been cured or discharged. While one patient has migrated.

425 new deaths were reported in the last 24 hours in the country due to COVID-19, taking the number of patients succumbing to the deadly virus to 19,693.

As per the Health Ministry, Maharashtra continues to be the most impacted state from the infection with 2,06,619 cases and 8,822 fatalities due to the virus. Tamil Nadu in second place has a total of 1,11,151 cases and 1,510 fatalities.

The national capital's COVID-19 cases are also nearing the 1-lakh mark with 99,444 coronavirus cases and 3,067 deaths.

The total number of samples tested up to July 5 is 99,69,662 of which 1,80,596 samples were tested yesterday, informed the Indian Council of Medical Research on Monday.

New Delhi, Feb 12: The Centre on Wednesday said the NRC data in Assam is safe even though some technical issues were visible and that will be resolved soon.

The Union Home Ministry clarification came in view of reports that data of the final list of the National Register of Citizens has been made offline from its official website.

"The NRC data is safe. Some technical issues are in visibility on cloud. These are being resolved soon," a home ministry spokesperson said.

The data was not available for a couple of days and it created panic in the public, mostly among the people excluded from the list as the rejection certificates were yet to be issued.

NRC State Coordinator Hitesh Dev Sarma accepted that the data has been made offline, but refuted the allegation of any "malafide" intent in it.

The cloud service for the huge set of data was provided by IT firm Wipro and their contract was till October 19 last year. However, this was not renewed by the previous coordinator.

So, the data got offline from December 15 after it was suspended by Wipro, Sarma said.

He said the state coordination committee had decided to do necessary formalities in its meeting on January 30 and wrote to the Wipro during the first week of February.

"Once Wipro makes the data live, it will be available for public. We hope people will be able to access it in the next 2-3 days," Sarma said.

The complete detail of exclusion and inclusion of bonafide Indian citizens in the NRC was uploaded on its official website http://www.nrcassam.nic.in after the final list was published on August 31, 2019.

The final NRC was published by excluding 19,06,657 persons. A total of 3,11,21,004 names were included out of 3,30,27,661 applicants.

After the earlier NRC State Coordinator Prateek Hajela relinquished the charge on November 11 following his transfer to home state Madhya Pradesh on a direction from the Supreme Court, Sarma was appointed in his place on November 9.
