Faulty data misled pilot in 2009 Air France plane crash: Report

Le Bourget (France), July 6: A combination of faulty sensors and mistakes by inadequately trained pilots caused an Air France jet to plunge into the Atlantic Ocean in 2009, killing all 228 people aboard in the airline's deadliest ever crash, French investigators said on Thursday.

Investigators are urging better instruction for pilots on flying manually at high altitudes and stricter plane certification rules as a result of a three-year investigation into what happened to Flight 447.

Airbus, manufacturer of the A330 plane, said in a statement that it is working to improve speed sensors known as pitot tubes and making other efforts to avoid future such accidents. Air France stressed the equipment troubles and insisted the pilots "acted in line with the information provided by the cockpit instruments and systems. .... The reading of the various data did not enable them to apply the appropriate action."


But the Bureau for Investigations and Analysis' findings raised broader concerns about training for pilots worldwide flying high-tech planes when confronted with a high-altitude crisis.

The report also could have legal implications: A separate French judicial investigation is still under way, and Air France and Airbus have been handed preliminary manslaughter charges.

The bureau's analysis lists a combination of "human and technical factors" behind the crash. The plane flying from Rio de Janeiro to Paris slammed into the sea during a nighttime thunderstorm on June 1, 2009.

Some families of victims felt investigators didn't focus hard enough on the equipment problems, saying the two pilots at the controls were doing what they could while faced with a barrage of inaccurate information.

Ice crystals that blocked the pitot tubes were the "unleashing event" that set off the plane's troubles, chief investigator Alain Bouillard said. The plane's autopilot shut off and the co-pilots had to fly manually, while a succession of alarms were going off. The captain was on a rest break.

In one fatal decision, the report says, one of the co-pilots nosed the Airbus A330 upward during a stall - instead of downward, as he should have - because of false data from sensors about the plane's position. Mr Bouillard said that was an "important element" of the cause of the crash.

He said the two pilots at the controls never understood that the plane was in a stall. He said only a well-experienced crew with a clear understanding of the situation could have stabilized the plane in those conditions.

"In this case, the crew was in a state of near-total loss of control," Mr Bouillard said.

Robert Soulas, who lost his daughter and son-in-law in the crash, says investigators said the flight director system indicated the "erroneous information" that the plane was diving downward, "and therefore to compensate, the pilot had a tendency to pull on the throttle to make it rise up."

However, the plane was in a stall instead. A basic maneuver for stall recovery, which pilots are taught at the outset of their flight training, is to push the yoke forward and apply full throttle to lower the nose of the plane and build up speed. But because the pilot thought the plane was diving, he nosed up.

Some families of people who died in the crash showed sympathy toward the pilots, saying they were dealing with bad equipment in an exceptionally challenging situation.

Mr Soulas noted that manufacturers had known for years about problems with the plane's speed sensors freezing over, but didn't order the faulty models systematically replaced until after the crash.

Pilot Gerard Arnoux said, "A normal pilot on a normal airliner follows" the signals on the flight director system, which tells them to go left, right, up or down.

Central to this accident is the fact that when the automation failed, the pilots were presented with conflicting information which was obviously incorrect, said William Voss, president of the Flight Safety Foundation in Alexandria, Virginia. But they were unable to look through this and understand what the aircraft was actually doing.

"Pilots a generation ago would have done that and understand what was going on, but (the AF447 pilots) were so conditioned to rely on the automation that they were unable to do this," he said.

"This is a problem not just limited to Air France or Airbus," Mr Voss said. "It's a problem we're seeing around the world because pilots are being conditioned to treat automated processed data as truth, and not compare it with the raw information that lies underneath."

The final report included a study of the plane's black box flight recorders, uncovered in a costly and extraordinarily complex search in the ocean depths.

Lais Seba, the mother of 31-year-old victim Luciana Clarkson Seba, said "it's going to be forever difficult" for survivors to deal with the loss of their loved-ones.

"We are surviving," she said. "We live one day at a time, with lots of pain, and always missing her."

Months after the outbreak of COVID-19 in Wuhan city of central China, families of those deceased, who contracted the contagious infection, stood in long queues at funeral homes demanding to receive the cremated ashes of their loved ones.

Now this has spurred questions about the actual tally of COVID-19 related casualties in Wuhan, in a renewed pressure on the Chinese government that is already struggling to control its containment narrative of the pandemic spread.

Chinese media outlet Caixin showed how trucks carrying 2,500 urns with the ashes of the deceased COVID-19 cases were being shipped in a funeral home last week. Another picture published revealed how 3,500 urns were stacked within these funeral homes. It is therefore unclear how many urns have been filled in.

According to media reports, workers at several funeral parlors declined to provide any details as to how many urns were waiting to be collected, saying they either did not know or were not authorised to share the number.

Some families said they had been forced to wait for several hours to pick up the ashes. The photos circulated as mass deaths from the virus spiked in cities across the west, including Milan, Madrid and New York, where hospitals were erecting tents to handle the overflow as global infections soar past 500,000, with 24,000 dead.

According to Chinese government figures, 2,535 people in Wuhan have died of the virus. The announcement that a lockdown in place since January would be lifted came after the country said its tally of new cases had hit zero and stepped up diplomatic outreach to other countries hard hit by the virus, sending some of them medical supplies.

But some in China have been skeptical of the accuracy of the official tally, particularly given Wuhan's overwhelmed medical system, authorities' attempts to cover up the outbreak in its initial stages, and multiple revisions to the way official cases are counted.

Residents on social media have demanded disciplinary action against top Wuhan officials.

Many people who died had Covid-19 symptoms, but weren't tested and excluded from the official case tally, Caixin said. There were also patients who died of other diseases due to a lack of proper treatment when hospitals were overwhelmed dealing with those who had the coronavirus.

There were 56,007 cremations in Wuhan in the fourth quarter of 2019, according to data from the city's civil affairs agency. The number of cremations was 1,583 higher than those in the fourth quarter of 2018 and 2,231 higher than the fourth quarter of 2017.

Two locals in Wuhan who have lost family members to the virus said online that they were informed they had to be accompanied by their employers or officials from neighborhood committees when picking up the urns, likely as a measure against public gatherings.

COVID-19 is affecting 199 countries and territories around the world. Over 664,000 coronavirus cases have been registered globally out of which 30,890 have succumbed to the infection.

Washington, Jul 14: Florida on Sunday reported a record 15,300 new coronavirus cases, the most by any state in a single day even as the coronavirus cases in the country have surged to 3,363,056.

The Washington Post reported that the huge number was result of both increased testing and widespread community transmission. The numbers shattered previous highs of 11,694 reported by California last week and 11,571 reported by New York on April 15.

Natalie E. Dean, an assistant professor of biostatistics at the University of Florida wrote that with Florida largely open for business, he doesn't expect this surge to slow.

Nationally, the conversation over reopening has become increasingly fraught amid the newly soaring case numbers, with much of the debate centering on whether schools should open their doors in the fall, reported the Post.

The Health workers in California and Texas too are facing an influx of COVID-19 patients where officials reported seven day averages for new cases - 8,664 and 9060 respectively.

According to the report, Florida has reported nearly 70,000 cases in last week alone, the most of any state.

Even though the COVID-19 cases are surging, Florida Gov. Ron DeSantis has stuck to an aggressive reopening plan with state officials recently ordering schools to reopen five days a week in the new academic year.

The state is also set to hold the Republican National Convention next month in Jacksonville's VyStar Veterans Memorial Arena, an indoor facility that seats about 15,000, reported Washington Post.

Seven-day averages for new cases -- considered a more reliable indicator of the virus's impact than single-day totals -- hit new highs in Alabama, Florida, Mississippi, Montana, North Carolina, Oklahoma and Puerto Rico.

Seoul, Jun 24: North Korea on Wednesday said leader Kim Jong Un suspended a planned military retaliation against South Korea, possibly slowing the pressure campaign it has waged against its rival amid stalled nuclear negotiations with the Trump administration.

Last week, the North had declared relations with the South as fully ruptured, destroyed an inter-Korean liaison office in its territory and threatened unspecified military action to censure Seoul for a lack of progress in bilateral cooperation and for activists floating anti-Pyongyang leaflets across the border.

Analysts say North Korea, after weeks deliberately raising tensions, may be pulling away just enough to make room for South Korean concessions.

Pyongyang's official Korean Central News Agency said Kim presided by video conference over a meeting Tuesday of the ruling Workers' Party's Central Military Commission, which decided to postpone plans for military action against the South brought up by the North's military leaders.

KCNA didn't specify why the decision was made. It said other discussions included bolstering the country's "war deterrent".

Yoh Sang-key, spokesman of South Korea's Unification Ministry, said Seoul was "closely reviewing" the North's report but didn't further elaborate.

Yoh also said it was the first report in state media of Kim holding a video conferencing meeting, but he didn't provide a specific answer when asked whether that would have something to do with the coronavirus.

The North says there hasn't been a single COVID-19 case on its territory, but the claim is questioned by outside experts.

Kim Dong-yub, an analyst from Seoul's Institute for Far Eastern Studies, said it's likely that the North is waiting for further action from the South to salvage ties from what it sees as a position of strength, rather than softening its stance on its rival.

"What's clear is that the North said (the military action) was postponed, not cancelled," said Kim, a former South Korean military official who participated in inter-Korean military negotiations.

Other experts say the North would be seeking something major from the South, possibly a commitment to resume operations at a shuttered joint factory park in Kaesong, which was where the liaison office was located, or restart South Korean tours to the North's Diamond Mountain resort.

Those steps are prohibited by the international sanctions against the North over its nuclear weapons programme.

The public face of the North's recent bashing of the South has been Kim Yo Jong, the powerful sister of leader Kim Jong Un, who has been confirmed as his top official on inter-Korean affairs.

Issuing harsh statements through state media, she had said the North's demolishing of the liaison office would be just the first in a series of retaliatory action against the enemy South and that she would leave it to the North's military to come up with the next steps.

The General Staff of the North's military has said it would send troops to the mothballed inter-Korean cooperation sites in Kaesong and Diamond Mountain and restart military drills in frontline areas.

Such steps would nullify a set of deals the Koreas reached during a flurry of diplomacy in 2018 that prohibited them from taking hostile action against each other.

Also condemning the South over North Korean refugees floating anti-Pyongyang leaflets across the border, the North said Monday it printed 12 million of its own propaganda leaflets to be dropped over the South in what would be its largest ever anti-Seoul leafleting campaign.

It wasn't immediately clear whether Kim's decision to hold back military action would affect the country's plans for leafleting. The North's military had said it would open border areas on land and sea and provide protection for civilians involved in the leafleting campaigns.

The North has a history of dialling up pressure against the South when it fails to get what it wants from the United States. The North's recent steps came after months of frustration over Seoul's unwillingness to defy US-led sanctions and restart the inter-Korean economic projects that would breathe life into its broken economy.

Nuclear negotiations between Pyongyang and Washington largely stalled after Kim's second summit with President Donald Trump last year in Vietnam, where the Americans rejected North Korea's demands for major sanctions relief in exchange for a partial surrender of its nuclear capabilities.
